To Apply for this Job Click Here A non profit supporting homeless clients is looking to add Case Managers to their growing team!The Housing Case Manager is responsible for supporting homeless individuals and families on their path to stable housing.
In this role, the Housing Case Manager will assist clients in accessing appropriate programs and resources, ensuring a successful transition to housing and greater overall stability.
This will require evaluating situations, making appropriate referrals, and diligently coordinating housing applications with the overall goal of empowering clients and helping them find realistic housing in which they can be successful.
The mission of the Coordinated Entry and Adult Shelter teams is to work with highly vulnerable individuals in need of intensive support to find stable housing.
The Housing Case Manager will work closely with the outreach team and be flexible by providing services through both intensive case management and drop-in assistance.
Qualifications:
HS Diploma 1
Year in Social Services Bi-lingual English/Spanish Experience working with vulnerable population Schedule:
Monday - Friday, 9am-5pmLocation:
