General Overview
Population Health Management (PHM) is an innovative model of care delivery that aims to improve the health of patients while lowering the overall costs of care. PHM includes best practices and tactics to enhance the patient experience, improve outcomes, and provide better organized and more personalized care to our patients. At Partners, we provide the resources and technology for our hospitals and community clinics to implement PHM across all phases of care.
As the Communication Project Manager, you will work closely with senior leadership and the Director of Communications to develop, coordinate, and execute projects that support PHM initiatives. You will serve as key support staff for a senior executive and prepare materials for internal and external presentations, reports, and other related materials. You will work collaboratively with leaders across PHM, Partners, and the broader hospital network. As a key member of the Communications team, you will also assist on a range of web and digital projects as well as other internal and external communications campaigns. The ideal candidate is a self-starter with strong leadership, communication, and organizational skills. PHM is a fast-paced work environment so you must able to multi-task and prioritize tasks effectively.
Principal Duties and Responsibilities
Responsible for development, execution, and coordination of projects that support PHM initiatives.
Perform background research and prepare materials (e.g. slides, briefing materials, talking points) to support needs of senior physician executive.
Critically synthesize issues and problem solve; independently assess calendar to determine needs and effectively triage to meet deadlines.
Oversee development and management of on-line resource and file sharing site for external website.
Lead strategic efforts for priority initiatives and translate organizational goals into executable projects.
Collaborate with senior leadership in PHM, including directors, analysts, and other leaders across Partners (e.g. Contracting, Community Health, and Communications).
Coordinate with PHM program teams to ensure information flows smoothly to relevant audiences, including management of program dashboard, standard communication materials, and reports.
Coordinate submission of award nominations and assist with select grant applications.
Write reports, proposals, and other relevant material to support PHM initiatives.
Ensure compliance with contract requirements for external communications (e.g. ACO website).
Assist the Communications team with special projects that may include writing, input, and coordination of web-based initiatives and other internal and external communications campaigns.
Monitor local and national developments about PHM and broader health care environment to stay informed.
Bachelor's degree required. Masters degree in public health, public affairs, health care management, health care communications, or related field preferred.
Experience writing, editing, and compiling reports, proposals, and related materials.
Minimum of five years of experience in health care or related field.
Skills/Abilities/Competencies
Outstanding communication and interpersonal skills; ability to present complex information both verbally and written to a wide range of audiences.
Ability to manage initiatives within a complex matrix organization, to persuade and engage physicians and staff at all levels of the organization.
Previous leadership experience, including operations and program development activities.
Solid writing, proofreading and editing skills, with excellent attention to detail and accuracy.
Demonstrated analytic abilities and skills; ability to interpreting and summarizing data.
Comfortable, flexible and able to thrive in a fast paced, rapidly changing health care environment.
A highly-organized self-starter, able to manage competing demands effectively and have the ability to effectively prioritize tasks and work within deadlines.
Capable of handling scheduled and impromptu assignments on a daily basis with accuracy and efficiency.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
